Check the following:

Order Type: Is it a Limit order set too far from market price?

Quantity: Is it aligned with exchange lot size?

Trading Mode: If in Paper mode, simulated fills might differ slightly

Try switching to a Market Order or adjust the price to match live bids/offers.
